Strong documentation protects residents, your facility, and your license. A refresher on the fundamentals that matter most in SNF settings.
Document in real time
Chart as close to the point of care as possible. Real-time documentation is more accurate, more defensible, and far less likely to be forgotten at the end of a busy shift.
Be objective and specific
Record observable facts, measurements, and resident statements in quotes. Avoid vague or subjective language that can be misinterpreted later.
Never skip the follow-up
If you document a change in condition, document the action taken and the response. An incomplete narrative is a liability.
